The following kinds of certificates are issued by the Consular Section of the Embassy:

Types of Certificates Fee in Euros
Registration of Indian Nationals Free (A photograph and Original passport should be submitted)
Registration of Child's Birth Certificate (should be done within one  year of birth of the child, otherwise delays should be expected) Application cannot be sent by post. Father should sign the application in the presence of the consular officer in the Embassy. 25 + 2
Death Certificate 1
Nationality Certificate (use Form EAP II) 20 + 2
Matrimonial Condition Certificate (use Form EAP II) 25 + 2
Certificate giving details of birth as extracted from Passport  (use Form EAP II) 20 + 2
Police clearance certificate (use Form EAP II) 20 + 2
Certificate in respect of Military Service (use Form EAP II) 20 + 2
Consular inscription Certificate (use Form EAP II) 20 + 2
Life certificate (use Form EAP II) Free
Driving Licence Declaration (use Form EAP II) 25 + 2

Documents Required for Transportation of a Dead Body to India

(i) Death certificate registered with the Portuguese authorities
(ii) Death certificate issued by the hospital describing the cause of death
(iii) Embalming certificate
(iv) Free Mortuary Traffic
(v) Medical sanitary certificate

All documents must be translated into English and notarized.
All original documents must accompany a set of photocopies.
Original and photocopy of passport of the deceased should be produced.
Identification document of the person registering the death with the Embassy, along with a copy should be submitted. The original will be returned to the applicant.
